• 2 routing exams and cleanings per 12 month period
• All routine x-rays for the entire 12 month period
• 2 fluoride treatments for members under 18 years of age

• All endodontic procedures- 15% discount*
• All prosthetic procedures (crowns, bridges, dentures, and veneers) 10% discount*
